About The Position
Come join us at Ozanam Hall Nursing Home of Queens! A Faith-Based, Nonprofit, Short-Term Rehabilitation and Skilled Geriatric Care facility sponsored by the Carmelite Sisters for the Aged and Infirm, we've served the community since 1971. We are a place of life, a place of caring, and more importantly, a true home for our residents. We are seeking a Social Worker for a TEMPORARY assignment.
Requirements
- Must have a Bachelor's Degree in Social Work or related field.
- One year of Social Work in a Nursing Home required.
- Must have MDS assessment experience.
Nice To Haves
- Experience using PCC is a plus.
Responsibilities
- Interview residents and their families.
- Provide assessments and counseling to residents and families.
- Develop care plans based on the individual need of residents.
- Provide crisis intervention as needed.
- Provide grief and supportive counseling.
- Coordination of discharge planning and case management.
