Requirements
- Master’s degree from an accredited social work program
- 2 years of experience in an acute or post-acute setting
Nice To Haves
- LMSW - Licensed Medical Social Worker - State Licensure in the state of Georgia
Responsibilities
- Promote patient and family wellness
- Improve care outcomes
- Ensure access to appropriate hospital and community resources
- Manage, support, and develop comprehensive transitional care plans for patients with complex and psychosocial needs in Inpatient and Emergency Department (ED) environments.
