About The Position

The AIDS Center provides HIV primary and specialty care, sexually transmitted disease screening and treatment, HIV biomedical prevention services, Hepatitis C care, and LGBTQ care at 2 ambulatory sites, the Center for Positive Living/ID Clinic and the Oval Center. These sites serve approximately 4,000 patients per year at about 35,000 visits, and are steadily growing. To meet the access, medical, and bio-psychosocial needs of the AIDS Center's complex populations living with or at high risk for HIV, Hepatitis C, and sexual transmitted infections, the AIDS Center leadership has committed to increasing access to comprehensive & culturally appropriate care according to best practices for destination clinics as outlined by NYC DOH/AI, increasing uptake of HIV treatment, and insuring the linkage and retention in care of those diagnosed with HIV and Hepatitis C. As a professional member of an interdisciplinary team, the AIDS Center Social Worker provides a range of direct and indirect services for patient but not limited to, intakes/reassessments/other requirements of Grant-funded programs, individual and group psychosocial support sessions, case management, and the identification and assessment of psychosocial problems and their resolutions. In addition, the social worker collaborates with the patient/family/team to effect safe and timely treatment plans. The Social Worker will be part of one or more grant-funded program to provided supportive services to individuals living with HIV. The Social Worker complies with the mission statements of Montefiore and Social Services, and the Code of Ethics of the national Association of Social Work. This position will directly support the new Enhanced Data 2 Care Team.

Requirements

  • Master's Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education.
  • New York State Licensed Master of Social Work required (LMSW)
  • One year of experience in a similar setting required
  • Knowledge required of HIV, STI, and HCV primary and secondary prevention
  • Knowledge of trauma informed care and NCLAS standards
  • Intimate familiarity with LGBTQ+ health care best practices
  • Intimate familiarity with health issues and best practices regarding people of color and those of low socioeconomic status
  • Experience working with managing grant funded and research-based programs
  • Experience providing individual and group psychosocial support sessions

Nice To Haves

  • Experience with EPIC and AIRS preferred

Responsibilities

  • Screen those patients whose diagnosis and social data indicate need for social service intervention, according to Social Work practice standards and hospital policy
  • Make psychosocial assessments utilizing all available data, including interviews with patient, family and conference with interdisciplinary associates
  • Ensure continuity of care through development, implementation and validation of discharge/treatment plans, in a timely manner an din collaboration with patient, family and interdisciplinary team.
  • Maintain social work professional conduct and practices
  • Participate in departmental programs, i.e., case conferences, quality improvement, statistical reporting, continuing education, staff meeting and other related assigned duties.
  • Complete psychosocial assessments for all patients enrolled into services during intake appointments and every 6 months reassessments to track patient progress.
  • Regular check-ins during medical appointments and in-between visits should be completed on a as-needed basis.
  • Provide individual and group psychosocial support sessionsg per requirements of grant-funded programs, needs of the AIDS Center, and Social Work standards
