CBI Home Health is looking to hire a Social Worker in the East Ottawa region. Be the connection patients need beyond the hospital. As a Social Worker in our Integrated Care Program, you’ll support smooth transitions home by addressing psychosocial needs, connecting patients to community resources, and collaborating with an interdisciplinary team to improve outcomes and reduce readmissions.

Requirements

• A Bachelor of Social Work or a Master of Social Work
• A registered member in good standing with the Ontario College of Social Workers and Social Service Workers
• Bilingual language skills in English and French are a MUST have

Nice To Haves

• 1 to 2 years of community experience is an asset
• Passionate, empathetic, use good judgement and apply critical thinking when making client decisions
• Excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
• Ability to travel within the Hawkesbury community as part of the CBI Health Home Health Services team
