About The Position

Part-Time Hospice Social Worker (BSW/MSW) at Crown Hospice. This role involves providing emotional support and counseling to patients and families navigating end-of-life care. The social worker will complete psychosocial assessments, develop individualized care plans, connect patients and families with community resources, and serve as an advocate and educator throughout the hospice journey. This position is part of an interdisciplinary team, collaborating with nurses, physicians, chaplains, and hospice aides to deliver patient-centered care. The role offers a flexible schedule and the opportunity to make a meaningful difference in the lives of patients and their families during a critical time.

Requirements

  • Bachelor's or Master's degree in Social Work (BSW or MSW).
  • Current Missouri Social Work license (or eligibility for licensure, as applicable).
  • Minimum one (1) year of healthcare experience (hospice or home health preferred).
  • Excellent communication, organization, and relationship-building skills.
  • Current CPR certification (or willingness to obtain).
  • Valid driver's license, reliable transportation, and automobile insurance.

  • Hospice or home health experience preferred.
