The Social Worker in this role provides essential social work services to patients and their families undergoing mental health treatment. This position functions as a float social worker, covering planned and unplanned absences of unit-based social workers as directed by the Senior Social Worker. The role involves covering a minimum of 6 beds, or as need dictates, and actively participating as a member of the interdisciplinary treatment team. Key responsibilities include developing and implementing patient treatment and discharge plans, conducting psychosocial and family assessments, and coordinating the discharge planning process for patients on Adult and Pediatric Inpatient Psychiatry units. The social worker will provide individual, family, or group treatment to support comprehensive discharge plans and address psychosocial problems that may impede care or discharge. Additionally, the role involves developing interagency relationships and mobilizing community resources to support patients and their families.

Requirements

Master's degree from an accredited school of social work required
LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required or If duties will be performed under OMH a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire required

Responsibilities

Provides social work services to patients and their families admitted for mental health treatment.
Covers planned and unplanned absences of unit based social workers as identified by the Senior Social Worker.
Covers a minimum of 6 beds, or as need indicates.
Participates in the development and implementation of the patient’s treatment and discharge plan as a member of the interdisciplinary treatment team.
Provides psychosocial and family assessments.
Coordinates the discharge planning process for patients hospitalized on Adult Inpatient Psychiatry units and Pediatric Inpatient Psychiatry units.
Provides individual, family, or group treatment that will contribute to the implementation of a comprehensive discharge plan.
Provides social work treatment in response to patient/family psychosocial problems which may impede care and/or discharge.
Develops interagency relationships and mobilizes community resources on behalf of the patients and/or their families.
Completes Psychosocial Risk Screen at time of admission and ongoing assessment throughout hospitalization.
Identifies and coordinates safe and timely discharge plan with treatment team, patients/families and ensures a comprehensive discharge plan.
Initiates and/or facilitates completion of Safety Plan with patient prior to discharge from hospital.
Identifies, contacts, and coordinates with outpatient providers/agencies to facilitate continuity of care and discharge planning for the patient.
Assesses agency involvement and mobilizes family and/or appropriate community resources to support the patient’s successful functioning post discharge.
Documents interventions in compliance with Division, Department and Hospital Policy.
Provides education to patients and families related to internal and community resources and referrals.
Advocates and supports patients and their families in their efforts to interact with complex social service agencies involved in their care.
Attends Social Work Division and Department Staff meetings, as required.
Participates in activities of the Social Work Division including but not limited to timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.
Subscribes to the Departmental goal of creating a culturally competent environment by treating patients, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
Participates in available cultural competence events and training appropriate to job duties.
May provide social work coverage and back-up as needed during the absence of co-workers in the inpatient services.
Performs other duties, as requested and assigned.
