About The Position

This position involves providing social work services to the elderly population, focusing on case management and ensuring clients can live independently. The role requires conducting assessments, developing care plans, connecting clients with resources, and collaborating with a multidisciplinary team. The social worker will also be responsible for case recording and reporting, crisis intervention, and adhering to all legal and departmental regulations.

Requirements

  • Bachelor’s degree required in social work, psychology, counseling, rehabilitation, gerontology or sociology.
  • Two years of experience required working with the elderly.
  • Two years of case management experience required.
  • Clinical assessment skills.
  • Knowledge of the community.
  • Ability to work with a multidisciplinary team.
  • Ability to work with clients/families in the home setting.
  • Knowledge of aging and programs for seniors.

Responsibilities

  • Evaluate potential clients as a total person and identify functional limitations that impede independent living.
  • Conduct in-depth assessments and reassessments covering psychosocial, rehabilitation, and environmental concerns.
  • Refer clients to special consultants for detailed evaluations and recommendations to improve their functional level.
  • Develop care plans, implement services, and conduct follow-up and monitoring of the plan.
  • Identify and develop support systems for the client.
  • Consult with the Health Practitioner.
  • Perform case recording and reporting.
  • Provide case management in accordance with departmental policy, program standards, and patient needs, including completing assessment evaluations of a client’s health and psychosocial status, identifying issues specific to age, diagnosis, and disabilities, participating as an active team member in care planning, implementing interventions and services utilizing community resources, maintaining and documenting regular contact with clients, identifying, assessing, and responding to crisis situations, and observing all legal, departmental, and/or hospital regulations.
