This position is responsible for coordinating and monitoring all MIHP services provided to assigned beneficiaries, including referrals and follow-up. The Social Worker will perform home and office visits, completing assessments and follow-ups according to protocol and providing supportive interventions as deemed necessary. They will coordinate participant enrollment and engagement, participate in interdisciplinary care planning to develop individualized Plans of Care, and support participants in identifying barriers to healthy pregnancy and infant development. The role also involves connecting patients with medical and community resources, maintaining compliance with Medicaid and MIHP documentation standards, and participating in quality assurance and improvement activities. Additionally, the Social Worker will conduct outreach and health promotion activities regarding MIHP.
