About The Position

The Social Worker shares the responsibility of identifying social and emotional needs of each client. The position assists with arranging services to meet the needs of residents, providing liaison between the client, family, facility staff, and the community. Assess patients and gather information from available sources, including family, agencies, friends, and others, for analysis. The Social Worker formulates and communicates treatment information to other members of the interdisciplinary treatment team; they then participate in the development of the patient's treatment plan, coordinate or provide services including individual, group, family, marital and/or group psychotherapy and discharge planning. Serves as a resource to staff and community regarding psychosocial issues. Participates in Clinic, Hospital, or departmental planning committees and teams.

Requirements

  • Master’s degree in social work from a school approved by the Council on Social Work Education
  • Licensed Clinical Social Worker (LCSW) or Licensed Social Worker (LSW)
  • American Heart Association (BLS for Health Care Providers)
  • Excellent interpersonal and communication skills.
  • Solid computer skills, including proficiency with Microsoft software.
  • Strong analytical and problem-solving skills, with the ability to be detail oriented.

Nice To Haves

  • 2 years of hospital social work or relevant fieldwork placement in mental health field or home care
