About The Position
Eddy Visiting Nurse & Rehab Association has a rewarding opportunity for a part time Social Worker responsible to manage a caseload of patients requiring social work support through in-home visits. Some patients/families are supported as well through virtual/phone based social work support either in place of in home visits or in combination with in home visits. Manages a caseload of patients in their homes: work with an interdisciplinary care team of nurses, therapists and aides conduct a comprehensive assessment of patient/family needs link individuals to the community resources they need to remain in their home or move to a higher level of care if necessary Once trained, enjoy the independence of starting and ending your day from home with a flexible work schedule Be on the front-line of the transitions in health care from inpatient settings to the community – home health care is the future!
Requirements
- NYS Social Work Licensure
- Minimum of 1 year of relevant experience in home care, the medical field, work with seniors, etc.
