Requirements

  • Trauma-informed care
  • Field-based behavioral and medical health support
  • Experience with substance use, mental health, housing insecurity, and barriers to care
  • Conducting mental health and substance use screenings
  • Providing harm reduction education and supplies
  • Offering brief interventions and crisis support
  • Coordinating referrals and follow-up planning
  • Documentation of assessments and referrals
  • Collaboration with community partners

Responsibilities

  • Provides trauma-informed, field-based behavioral and medical health support to individuals impacted by substance use, mental health, housing insecurity, and barriers to care.
  • Conducts mental health and substance use screenings.
  • Provides harm reduction education and supplies.
  • Offers brief interventions and crisis support (including de-escalation support).
  • Coordinates referrals and follow-up planning to behavioral health, medical health and social services (such as mental health, recovery, medical care, housing, counseling, etc.).
  • Supports harm reduction education.
  • Ensures accurate documentation of assessments and referrals in compliance with legal requirements.
  • Contributes to student training and continuity of care through collaboration with community partners.
