About The Position

We are currently looking for a dedicated Social Worker to join our team and make a meaningful impact by offering emotional support, advocating for residents' needs, and guiding families through the complexities of both short-term and long-term care.

Requirements

Bilingual in English and Spanish
Strong communication, interpersonal, and organizational skills.
Compassion, patience, and a genuine passion for helping others.
Ability to work independently and collaboratively as part of a team.

Nice To Haves

Previous experience in long-term care or healthcare settings is preferred but not required.

Responsibilities

Assess the social, emotional, and environmental needs of residents and their families.
Develop and implement individualized care plans in collaboration with the healthcare team.
Provide emotional support, counseling, and crisis intervention to residents and families.
Assist new residents with the transition process and connect families with available resources.
Advocate for residents’ rights, ensuring they are treated with dignity and respect.
Help residents access community resources and support services as needed.
Maintain thorough and accurate documentation of social work interventions and resident progress.
Participate in care plan meetings and collaborate with the multidisciplinary team.
Ensure compliance with state and federal regulations related to social work services.
