About The Position

This role provides clinical services to patients and families, addressing psychosocial, environmental, age-specific, and cultural issues to maximize well-being and the effective use of healthcare and community resources. The position involves conducting psychosocial assessments, providing intervention and treatment, and offering clinical services within the Health Center clinic. Experience in CBT is preferred. The role requires effective collaboration within an interdisciplinary healthcare team, consultation to other disciplines, and participation in departmental and health center committees.

Requirements

  • Master's Degree Social Work required
  • Clinical Experience in a medical setting 2-3 years required
  • Licensed Independent Clinical Social Worker [LICSW - State License] required
  • Master's Degree Related Field of Study required
  • Licensed Mental Health Counselor [LMHC - State License] required
  • Related experience 3-5 years required
  • Demonstrated experience working successfully with people of various backgrounds and lifestyles in a manner that engages ongoing self-reflection and continual learning required
  • Experience negotiating, advocating, and navigating systems required

Nice To Haves

  • Experience and training in CBT preferred
  • Fluency in Spanish preferred
  • Experience working with Child/Adolescents preferred

Responsibilities

  • Provides clinical services with patients and families that address psychosocial, environmental, age specific and cultural issues in order to maximize emotional, social and physical well being and the effective/efficient use of health care and community resources.
  • Conducts psychosocial assessments, provides intervention/treatment as indicated.
  • Provides clinical services in the Health Center clinic.
  • Works effectively as part of an interdisciplinary health care team.
  • Collaborates with other health care professionals.
  • Provides social work consultation to other disciplines within the Hospital and community.
  • Participates on Departmental and Health Center committees as needed.
  • Screens patients who request service, are referred, or meet Unit screening criteria for identifying patient needs.
  • Formulates diagnosis, disposition, and treatment plans.
  • Employs clinical services such as individual (adult, adolescent and child), couples, family, and group therapy; crisis intervention and some case management.
  • Advocates on behalf of patients and/or their families to gain access to services and resources.
  • Facilitates the appropriate and efficient use of Hospital resources.
  • Presents and/or discusses clinical work in formal and informal settings and seminars.
  • May participate in research projects.
  • Collaborates and consults with patient, families, professionals, and providers inside and outside the Hospital and School.
  • Refers to and collaborates with interdepartmental specialists and programs, outside agencies, facilities, payors, and other relevant community resources.
  • Takes responsibility for own administrative duties, including timely and appropriate documentation in patient medical records and accurate reporting of time worked.
  • Attends and participates in unit meetings, division meetings, and staff meetings at the Hospital and High School.
  • Adheres to the profession’s clinical standards and code of ethics.
  • Adheres to the Department and Hospital standards, policies and procedures.
  • Promotes Departmental and Hospital public relations.
  • Participates in supervision as appropriate to his/her level of professional development.
  • May assist with or supervise undergraduate and graduate social work students and staff.
  • Is responsible for own professional development.
  • Attends pertinent continuing education and professional activities related to job responsibilities and career goals.
  • Keeps current with social work and health care developments pertinent to job and profession.
  • Complies with licensure requirements.
