About The Position

This role involves performing psychosocial assessments and providing direct social work services to patients and their families. The Social Worker will participate in formulating and implementing discharge plans, identify unmet psychosocial needs and develop programs to address them, and engage in educational activities. Key responsibilities include evaluating new admissions, preparing comprehensive care plans, participating in interdisciplinary team meetings, and maintaining thorough documentation of all patient and family interactions and assessments. The role requires employing clinical skills in counseling individuals and families regarding coping with illness, placement, and discharge planning, while also incorporating knowledge of community resources and entitlement programs. The Social Worker will negotiate community systems, collaborate with the interdisciplinary team, and continuously evaluate treatment plans to ensure they meet required standards. Additionally, the role requires utilizing knowledge of financial, reimbursement, and regulatory systems, completing required forms, anticipating case management needs, and identifying unmet psychosocial needs through collaboration with staff and the development of new programs. The Social Worker will also participate in group functioning, coordinate family groups, and represent the hospital department when meeting with community agencies.

Requirements

  • Master’s Degree in Social Work, required.
  • Licensed Master Social Worker (LMSW), required.
  • Minimum of three (3) years social work experience, including experience in an acute care hospital, required.
  • Specialized experience may be required, depending on area of assignment.

Responsibilities

  • Performs psychosocial assessments of patients and families.
  • Provides introduction and orientation to newly admitted patients/families.
  • Evaluates new admissions by interviewing patients and families, contacting other agencies, if indicated, and reviewing available information.
  • Prepares psychosocial assessments of patients and families using knowledge and understanding of the needs of the patient population.
  • Prepares comprehensive care plans.
  • Participates in interdisciplinary team meetings.
  • Maintains documentation of initial contacts with patient/families and of psychosocial assessments.
  • Formulates and implements psychosocial treatment plans.
  • Employs clinical skills in counseling individuals and families regarding coping/adjusting to illness and placement and discharge planning.
  • Formulates and implements discharge plans.
  • Establishes and maintains professional relationship with patients and families.
  • Incorporates knowledge of community resources and entitlement programs in planning.
