Requirements

• MSW required
• LSW or LCSW is also required or must be obtained within 2 years of hire date.

Nice To Haves

• Two years’ experience in the health care setting preferred.

Responsibilities

• Collaborates with hospice interdisciplinary team, hospitals, and care facilities to develop comfort-focused plans of care, complete comprehensive assessments, and evaluate ongoing interventions.
• Collaborates with patients, families, and case managers during hospital discharge planning to ensure safe and appropriate transitions of care.
• Conducts joint home visits with admissions nurses when psychosocial complexity is present (e.g., patients living alone, lacking caregivers, or experiencing mental health/substance use concerns).
• Performs informational and follow up visits for patients in hospitals, facilities, and home settings.
• Supports initial response for urgent referrals needing immediate response.
