To help share our mission and expand our community impact, Vitra Health is seeking a Social Worker who is passionate about community engagement and helping people access the care and resources they deserve. As a Vitra Health Social Worker, you will provide comprehensive psychosocial support, care coordination, and resource navigation for patients with complex medical and social needs. Services are provided under the supervision requirements of applicable regulations and organizational policies, and in alignment with interdisciplinary care teams. This role is ideal for someone who has strong Knowledge of healthcare systems, community resources, and social service programs.

Requirements

  • Master’s degree in social work (MSW) from a CSWE-accredited program
  • Active Social Work license in the state of Ohio
  • Minimum of 1 year of experience in a healthcare or clinical social work setting preferred
  • CPR and First Aid certification (or willingness to obtain)

Responsibilities

  • Provide counseling and psychosocial support to patients with complex medical and behavioral health needs
  • Assist patients in understanding, coping with, and managing their diagnosis and treatment plans
  • Collaborate with the interdisciplinary care team to develop, implement, and evaluate individualized care plans
  • Coordinate community-based services, including home care, medical equipment, transportation, and follow-up care
  • Monitor patient progress and adherence to treatment plans and medical recommendations
  • Provide counseling and support to families and caregivers to help them understand diagnoses and manage care needs
  • Conduct psychosocial assessments and participate in care planning and decision-making processes
  • Connect patients with community resources, including insurance programs, financial assistance, and social service agencies
  • Assist patients with eligibility determination and completion of required documentation (e.g., insurance, benefits forms)
  • Conduct in-person outreach visits in community and provider settings to support engagement, assessment, and care coordination
  • Fosters a culture of customer service and commitment to quality care
  • Serves as a brand ambassador for Vitra reflecting our vision, mission, and values
  • Shows genuine interest and compassion for the communities we serve and commitment to the diversity of our clients and team members
  • Mentor and supports team members
  • Complete other tasks as assigned
