Social Worker - TEMPORARY
About The Position
Ozanam Hall Nursing Home of Queens is seeking a Social Worker for a temporary, full-time assignment. The role pays $40 per hour and follows a Monday–Friday schedule. Under the supervision of the Director of Social Work, the Social Worker assists residents in attaining or maintaining the highest practicable physical, mental, and psychosocial well-being, and helps them navigate the health care system. The position involves interviewing residents and their families, conducting assessments and counseling, developing individualized care plans, providing crisis intervention and grief and supportive counseling, and coordinating discharge planning and case management. Requirements
- Bachelor’s degree in Social Work or a related field.
- One year of Social Work experience in a nursing home.
- MDS assessment experience.

- Experience using PCC.
Responsibilities
- Interview residents and their families.
- Provide assessments and counseling to residents and families.
- Develop care plans based on residents’ individual needs.
- Provide crisis intervention as needed.
- Provide grief and supportive counseling.
- Coordinate discharge planning and case management.
- Assist residents in attaining or maintaining the highest practicable physical, mental, and psychosocial well-being.
