About The Position
The Social Worker provides medical social work services to hospice patients and their families. The role supports patients and families with health-related financial, social, and emotional concerns and helps them develop effective coping skills. Services are provided in accordance with the plan of care, with timely communication of clinical status changes and identified environmental risks to the healthcare team and care center office staff. The Social Worker also educates patients, families, and team members about medical social work interventions and the treatment plan, and performs care-planning, transfer, and discharge functions.
Requirements
- Master's or doctorate degree from a school of social work accredited by the Council on Social Work Education.
- One year of social work experience in a health care setting.
- Meet state and federal requirements for the position.
- Current CPR certification.
Responsibilities
- Assist patients and families with health-related financial, social, and emotional concerns and effective coping skills.
- Provide social work services in accordance with the plan of care.
- Communicate changes in a patient's clinical status to healthcare team members and care center office staff in a timely manner.
- Educate patients, family members, and other team members on medical social work interventions and the treatment plan.
- Perform care-planning, transfer, and discharge functions.
- Identify risks or threats to patient health in the patient's environment and promptly implement appropriate interventions to reduce risks.
- Communicate identified patient risks or threats to the physician and other healthcare team members to coordinate care effectively.
- Perform other duties as assigned.
