About The Position

The Social Worker delivers and coordinates comprehensive psychosocial care for selected patients and their families. This includes conducting biopsychosocial assessments, implementing appropriate interventions and treatment plans, and identifying high-risk psychosocial factors that may affect health outcomes. The role also involves collaborating with interdisciplinary teams and providing social work consultation within the healthcare setting and the community to support optimal patient care.

Requirements

  • Masters: Social Work (Required)
  • A minimum of one (1) year of social work experience in a health care setting.
  • Must display a high level of flexibility, adaptability and organizational skills.
  • Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
  • Must be able to prioritize and organize time and functions.
  • Must communicate effectively and respectfully with patients, families, and all levels of staff and management.
  • Required to provide conscientious commitment to assigned work, work schedule, honesty, teamwork, assisting others to bring about positive outcomes.
  • Must be able to recognize and manages risks, resolve issues and problems as soon as they arise, report and document adverse events and reportable conditions.
  • Must demonstrate the ability to provide social work services to patient, family members and significant others in four areas: 1) psychological functioning, 2) adaptation to illness, injury, disability or life event, 3) psychosocial assessment/evaluation and 4) facilitation services such as referral community agencies financial planning, and provision of information and resource knowledge.
  • Required to identify and intervene for suspected abuse including sensitivity to age specific concerns; physical/emotional/sexual abuse of children and adolescents; domestic abuse for adults; and caretaker/institutional abuse for senior adults.
  • Must be able to manage crisis situations, demonstrating ability to quickly appraise the crisis situation and select appropriate intervention.
  • Able to document direct service in patient’s medical record after each significant encounter.
  • Must demonstrate proficiency in use of the electronic medical record.
  • Must be able to demonstrate good problem-solving skills, root-cause analysis, and principles of clinical performance improvement.

Responsibilities

  • Develops comprehensive discharge planning and/or bio-psychosocial assessments responsive to age-appropriate and cultural needs and concerns.
  • Employs a range of clinical interventions such as adjustment to illness counseling, crisis intervention, care coordination, information and referral, and safety planning.
  • Advocates on behalf of patients and families to gain access to services and resources.
  • Supports the department's initiatives, mission and goals and may take on additional responsibilities at the discretion of leadership.
  • Provides assistance with medications, transportation, and advance directives.
  • Assesses patient/family learning needs, styles and readiness. Educates patients/families based on treatment plan, identifies barriers to care, diversity issues and learning styles.
  • Supports patients with mental health needs during the episode of care, discharge planning to both inpatient and outpatient mental health providers.
  • Participates in case reviews to evaluate case management and progress.
  • Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
  • Identifies and refers patients for specialized services, such as drug and alcohol treatment, legal services, financial and employment counseling, and housing support.
  • Provides psychosocial support and interventions, particularly for individuals with chronic and/or complex medical and behavioral health conditions screened as high or moderate risk.
  • Provides outreach and coordination assistance with community resources and agencies, and community-level advocacy on behalf of hospitalized patients.
  • Refers to and collaborates with Memorial Healthcare System Social Determinants of Health (SDOH) Hub.
  • Performs all aspects of psychosocial discharge planning for chronic conditions and/or long-term custodial care needs for inpatient discharge, community services, outpatient care and any other applicable setting.
  • Provides assessment and intervention in domestic violence and abuse situations.
