About The Position

The Social Worker will join the Social Services team on the Medical Specialty team, serving patients with significant mental health needs and acute medical issues. The role provides acute care patients with services such as discharge planning, care coordination and navigation, resource connections, supportive listening, advance care planning completion, and long-term care planning. The Social Worker assesses complex biopsychosocial needs and provides interventions in support of individualized treatment plans. The position also serves as a consultant and leader in comprehensive care throughout the system while supporting organizational strategies and goals. This is a full-time position working 40 hours per week during business hours Monday through Friday, between 7:30 a.m. and 5 p.m. Weekend work is not scheduled; the role participates in a rotation for one holiday per year. The position is located at the La Crosse main campus. Starting wage is $25.95 per hour, with additional compensation for comparable experience.

Requirements

Bachelor’s degree in Social Work or a related field.
Wisconsin Social Worker license required.

Nice To Haves
