Works as part of the Mental Health and Social Services team to provide clinical assessment, referral, and placement services to homeless guests entering emergency shelters. The role involves assessing guests for mental illness, identifying high-risk behaviors, providing crisis intervention, and facilitating a path out of shelter through diversion plans or referrals to housing. The Social Worker collaborates with internal and external agencies, including clinicians, hospitals, mental health facilities, and substance abuse treatment programs, to ensure clients receive appropriate services. Responsibilities also include assisting with eligibility applications, coordinating services, developing referral sources, maintaining accurate records, compiling reports, attending meetings and trainings, and providing psycho-educational groups.
