About The Position

This position involves providing comprehensive social work services to patients within the Marshall Psychiatry department at Mildred Mitchell Bateman Hospital. The role focuses on assessing psychosocial factors impacting patient care, providing counseling and support, and coordinating with multidisciplinary teams and community resources.

Requirements

  • Master's degree in a clinical mental health field (counseling, psychology, or social work).
  • LPC or LICSW license required.
  • Must be independently licensed in the state of West Virginia in counseling, psychology, or social work.

Nice To Haves

  • Two years of experience preferred.

Responsibilities

  • Interviews patient and family members to obtain information about home environment, family relationships, and health history.
  • Performs accurate social work assessment and documents findings in the patient's electronic health record.
  • Provides support services and counseling to patients with psychosocial issues related to their illness, family illness, and grief reactions.
  • Contributes to a multi-disciplinary approach to treatment plans, coordinating with and providing feedback to other medical staff.
  • Reviews socioeconomic issues impacting patients' care and explains their impact.
  • Educates and supports patients and families in exploring care options, including home health services, hospice, assisted living, and skilled nursing facilities, and assists with admission paperwork.
  • Educates and supports patients and families on healthcare system services (e.g., Medicare, Medicaid, Medicare Part D, vocational rehabilitation) and refers them as appropriate.
  • Maintains current information on local healthcare and community services.
  • Serves as a liaison between the department and community service organizations.
  • Performs other duties as assigned or requested.
