About The Position

This PRN position involves conducting comprehensive psychosocial assessments and developing individualized care plans to address residents' emotional, social, and discharge planning needs. The role includes providing counseling, intervention, and ongoing support to residents and their families while advocating for resident rights and quality of life. Collaboration with an interdisciplinary care team is essential for coordinating services, facilitating care conferences, and ensuring resident-centered care. Additionally, the Social Worker will connect residents and families with community resources and post-discharge support services to promote successful care transitions.

Requirements

Bachelor or Master of Social Work (BSW or MSW)
Current, unencumbered MA LSW, LCSW, or LICSW license
Ability to communicate fluently in English – both verbally and written

Nice To Haves

Prior experience in a long-term care setting strongly preferred

Responsibilities
