About The Position

Roze Room is a leading provider of Hospice and Palliative Care, celebrating 25 years of service to Southern California communities. This is a Full-Time Medical Social Worker (MSW) position focused on Hospice & Palliative Care within the Ventura County Service Areas. The role involves providing psychosocial assessments, emotional support, counseling, and resource coordination for patients and their families, addressing social, emotional, cultural, and practical challenges during serious illness and end-of-life care.

Requirements

• Master’s Degree in Social Work (MSW) from an accredited program
• Comfortable using EMR systems and computer applications
• Valid California driver’s license, current auto insurance, and reliable transportation

Nice To Haves

• Hospice, palliative care, home health, or healthcare experience preferred
• Strong communication, organizational, and interpersonal skills
• Spanish-speaking preferred but not required

Responsibilities

• Conduct comprehensive psychosocial assessments and ongoing evaluations
• Provide counseling and emotional support to patients, families, and caregivers
• Assist with advance care planning and community resource referrals
• Collaborate with physicians, nurses, chaplains, and other hospice team members
• Maintain accurate documentation in the electronic medical record (EMR)
• Support patients and families through grief, loss, and end-of-life transitions
