About The Position

Arroyo Grande Community Hospital ARU is seeking a Social Worker for their Acute Rehabilitation unit. This per diem position offers a wage scale of $45.00-$55.00 per hour. The role involves assessing and planning interventions to help patients cope with social, emotional, economic, and environmental problems, contributing to the mission of making communities healthier.

Requirements

• Bachelors required
• Board Certified Social Worker, or related field (i.e. counseling)
• Handle With Care within 30 days of hire.
• Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
• Must be able to work in a stressful environment and take appropriate action.

Nice To Haves

• Masters preferred

Responsibilities

• Assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress.
• Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes.
• Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.
