This role provides supportive services and counseling on healthcare and home care programs and services. The Social Worker serves as a member of the interdisciplinary team, assisting with social, emotional, and economic concerns of patients/clients/residents and their families/caregivers to help them achieve or maintain an optimal level of functioning by coordinating and planning programs. This includes providing crisis intervention, assisting families in understanding medical situations, coordinating healthcare programs, and responding to suspected abuse, neglect, or violence in accordance with ethical policies, procedures, and state laws. The role involves developing appropriate plans of care, obtaining resources from social, health, and human services agencies, and providing referrals, information, and education. Professionalism is demonstrated through participation in care conferences and transitional rounding, acting as an advocate, and utilizing time effectively. The position requires critical thinking, problem-solving, decision-making, and strong written and verbal communication skills to build rapport with patients, families, communities, and healthcare teams. Independent judgment is expected, and active participation with healthcare teams is essential. Depending on the department, social services may be provided for donors and transplant recipients.
