About The Position

This Social Worker position functions as an integral member of the Care Management Team, leading the coordination of behavioral health, substance and alcohol disorders, and comprehensive social work services for Multi-Visit Patients (MVPs) of the OSU Emergency Departments. In collaboration with the Care Management Team, the social worker connects patients to necessary behavioral health services, substance and alcohol use disorder treatment, and other supportive services to improve patient outcomes and reduce high utilization of acute care services. The Social Worker will collaborate closely with OSU behavioral health resources and community partners to facilitate and enhance access to appropriate behavioral health services for MVPs many which have complex medical needs and significant non-medical social needs.

Requirements

  • Master of Social Work from an accredited Graduate School of Social Work required.
  • State of Ohio Social Work licensure (LSW or LISW) required.
  • 1 year of relevant experience required.

Nice To Haves

  • Experience in a physician’s office or other primary health care setting desirable.
  • 2-4 years of relevant experience preferred.
