About The Position

The Social Worker uses experience, knowledge, judgment, and skills to provide social services to patients and families in both complicated and uncomplicated situations. The Social Worker maintains patient confidentiality and promotes patient rights as supported by agency, state, and federal directives. Identifies cultural diversity and lifestyle choices and non-judgmentally accommodates for such differences. Additionally, they provide consultation to other hospice staff and volunteers while consistently demonstrating the Hospice Savannah values and compliance with the organization’s service excellence standards.

Requirements

• Graduate of an accredited school of social work with a Master's degree in Social Work required.
• Licensed Social Worker required.
• Minimum of 2 years social services experience.
• Valid driver's license, automobile insurance, and reliable transportation required.
• Excellent communication skills (written and verbal).
• Ability to successfully interact with a diverse patient population, families, physicians, Hospice Savannah staff, and the community at-large.
• Excellent customer service and documentation skills required.
• Computer proficiency in Microsoft Office, including Word and Outlook, and database knowledge required.

Nice To Haves

• Family work experience preferred.
• Healthcare experience preferred.

Responsibilities

• Provide social services to patients and families.
• Maintain patient confidentiality and promote patient rights.
• Identify and accommodate cultural diversity and lifestyle choices.
• Provide consultation to other hospice staff and volunteers.
• Document patient/family encounters accurately, timely, and concisely using a computer.
• Participate in the rotating on-call schedule.
• Perform patient visits utilizing their personal vehicle and may require extensive driving.
