About The Position

Hospital Sisters Health System (HSHS) is seeking a Social Worker to join their team. The Social Worker will coordinate and provide social services for patients of all ages in the patient's care setting. Social workers play an essential role in many aspects of patient care, including helping patients and their families navigate the medical system, assessing and monitoring patients' and family members' mental and emotional health, providing short-term counseling and therapy, and communicating patient needs and concerns to the larger medical team. This position is part of the Care Management department and focuses on Behavioral Health/Social Work. The schedule is 32 hours per week, with every 5th weekend rotation, at St. Vincent's Hospital in Green Bay, WI.

Requirements

  • Master’s Degree in social work is required.
  • Licensed as a Certified Social Worker (CSW) in the state of Wisconsin or Licensed Social Worker (LSW) in the state of Illinois is required.
  • Basic Life Support (BLS) is required.
  • In roles which travel is required, a valid driver's license, automobile insurance and own transportation is required.

Nice To Haves

  • Bachelor's degree in social work plus five years of social work or social service clinical experience may be considered in lieu of a master's degree.
  • Prior experience in social work or past social service clinical experience directly transferable to a healthcare environment is preferred.
  • 2 years of home care, hospice, discharge planning or related experience is preferred.
  • Wisconsin: Licensed as a Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) is preferred.
  • Illinois: Licensed as a Licensed Clinical Social Worker (LCSW) is preferred.
