This role is for a Social Worker within the Adult Partial Hospital Program (PHP). The social worker is responsible for providing primary therapy to patients 18 years and older, conducting psychosocial assessments, treatment and discharge planning, crisis intervention, and maintaining the therapeutic milieu. They will also be responsible for assessing and managing patient safety, potentially leading therapeutic groups, conducting intake assessments, and developing relationships with community agencies. The social worker will be an integral part of a multidisciplinary team for case review and will document treatment in the electronic medical record.
