The Social Worker supports the interdisciplinary program/team, under the direction of the Patient Care Manager, in achieving program, patient, and resident goals. The Adult Bleeding Disorders Program of BC and the Adult Red Cell Disorders Program of BC, based at St. Paul’s Hospital, provide specialized, patient-centred care for individuals living with inherited bleeding and red cell disorders, including hemophilia, von Willebrand disease, sickle cell disease, and thalassemia. These provincial interdisciplinary programs provide comprehensive diagnostics, coordinated treatment planning, and long-term support for patients across BC and Yukon. As a Social Worker within these programs, the role supports patients and families navigating complex hematologic conditions by addressing psychosocial needs, improving access to care, and contributing to coordinated, interdisciplinary care.
