The social worker is responsible for providing social work services to Pediatric and OB/GYN patient care units on weekends and holidays or weekdays in instances when the unit's assigned social worker is absent.

Requirements

  • Master's degree from an accredited school of social work required Or equivalent combination of education and experience
  • LMSW - Licensed Medical Social Worker - State Licensure within 1-1/2 years required or If duties are performed under OMH a minimum of a limited permit (not to exceed 1 year) or NYS Licensure upon hire required.

Responsibilities

  • Responds to requests for social work consults or interventions from GCH and OB/GYN inpatient units.
  • Assesses patients identified in need and provides social work interventions and supportive counseling to address identified risks and SDOH.
  • Manages situations involving disruptive visitors in close collaboration with DPS, health care team and social work supervisor.
  • Identifies and manages all cases of suspected child abuse and neglect not covered by other pediatric specialty social workers
  • Provides discharge planning in collaboration with the care team for patients, assuming the primary coordinator role when there is child protective involvement, alternate discharge of a minor and transfer to another facility including acute to acute.
  • Documents social work activity in patient's medical record including completion of the pediatric psychosocial assessment, palliative care and Craft screen as appropriate.
  • Participates in activities of the Social Work Division including but not limited to meetings, time reporting, and professional development, mandatory in-service training programs and UHS review.
  • Participates in professional development with relevance to the position
  • Provides handoffs to week day social workers or those in the outpatient after care settings.
