We are looking for a Social Worker to coordinate and perform administrative functions related to resident admissions and discharges in Care Services (healthcare), and to assist in the organization and planning of Social Services for our community.

Requirements

Bachelor’s degree in Social Work or closely related human services field required
1-2 years related experience in a Healthcare setting.
Two years’ experience in a similar social work position required.
Ability to read, write, speak and understand English fluently.
Ability to meet or exceed the company’s attendance and punctuality standards.
Ability to use Electronic Records and miscellaneous software and office equipment.
Ability to understand and follow directions as given.
Ability to work with minimal supervision.

Nice To Haves

Two years of long-term care or rehabilitation experience desired.

Responsibilities

Assists with the responsibilities and functions delineated in the discharge plan.
Ensures that the administrative and clerical functions related to the admission and/or discharge of residents is carried out to completion by coordinating efforts with the Healthcare Team Members.
Maintains appropriate contact with hospitals, doctors and outside agencies to facilitate admissions.
Coordinates all forms needed for admissions.
Maintains state required resident roster.
Coordinates financial updates for private pay residents.
Completes appropriate section of the MDS.
Participate and attend all in-service training and education programs as scheduled.
Other duties as assigned.
