Requirements

  • Licensed in the state of Idaho as a Licensed Social Worker (LSW), Licensed Master’s Social Worker (LMSW), or Licensed Clinical Social Worker (LCSW).
  • BLS CPR certified
  • Meet all licensing requirements related to your clinical license

Nice To Haves

  • Home Health and Hospice experience preferred, but not required

Responsibilities

  • Work with patients, their families, and other health care professionals in the patients’ place of residency.
  • Perform visits and complete coordination of care.
  • Follow agency, state, and federal policies and procedures.
  • Work in an interdisciplinary team and coordinate with other agencies in the community to connect care for patients and their families.
