About The Position

This role is responsible for promoting a safe, cooperative, and professional healthcare environment to ensure optimum patient care. The Social Worker adheres to all hospital policies and procedures, prioritizing customer service by treating patients, visitors, and co-workers with courtesy and tact. A key aspect of the role involves interviewing patients and families to gather psychosocial information, assessing patient functioning related to safety and discharge planning needs, and formulating a clinically realistic and sensitive assessment and treatment/discharge plan. The Social Worker participates in multidisciplinary rounds and meetings, provides direct social work and discharge planning services, including community resource advocacy, and maintains professional rapport with community agencies. All work activities are documented in the patient's medical chart and Allscripts according to guidelines. The role requires engaging with patients and families with cultural sensitivity, maintaining patient confidentiality, and counseling individuals regarding lifestyle changes and coping patterns. The Social Worker is responsible for timely completion of monthly statistics and attending all scheduled department meetings. They also provide age-appropriate conversation and demeanor when interacting with patients and families, and offer rotating weekend coverage in various departments, including the Pediatric Emergency Department, Nursery, and Labor and Delivery. The position involves attending scheduled meetings, participating in White Board Rounds, discharge planning meetings, and patient conferences, with supporting documentation in AllScripts and Weekend Log. Additionally, the Social Worker attends scheduled supervision and obtains consultation when needed, and performs other assigned duties.

Requirements

  • Master’s Degree in Social Work from an accredited school of social work.
  • New York State Certification (LMSW). [Employee must either: 1) Possess at time of hire or 2) obtain within 6 months of start date.]
  • Two (2) years’ experience in a hospital setting, and related to the department to which the worker will be assigned.

  • Makes Customer Services a priority, treating customers (patients, visitors and co-workers) in a professional manner exercising courtesy and tact.
  • Effectively communicates with patients and others about SBH services in their area(s) of expertise or responsibility and ensure that concerns, questions or issues reach the right person(s) in SBH in a timely fashion.
  • Interviews patient and/or family to obtain psychosocial information and assess the patient’s functioning relative to safety and/or discharge planning needs.
  • Formulates an assessment and treatment/discharge plan that will highlight a clinically realistic, and sensitive understanding of the patient’s needs.
  • Participates in scheduled multidisciplinary rounds/meetings.
  • Provides direct social work/discharge planning services including community resource advocacy as well as keep ongoing professional rapport with community agencies as needed.
  • Documents all work activity in the patient’s medical chart and Allscripts in accordance with departmental and regulatory guidelines.
  • Engages with patient and/or family members with respect to cultural differences and sensitivity to individuality.
  • Demonstrates consistent attention towards patient confidentiality.
  • Counsels patients, families, and significant others in the areas of expectation regarding life style changes and functional coping patterns.
  • Completes and turns in monthly statistics timely.
  • Attends all scheduled Social Work/Discharge Planning Department meetings.
  • Speaks to patients, families and significant others with age appropriate conversation and demeanor.
  • Provides social work rotating weekend coverage in Pediatric Emergency Department, Nursery, Labor and Delivery and other areas, as necessary.
  • Attends scheduled meetings, participates in White Board Rounds, discharge planning meetings and patient conferences, etc. as needed.
  • Provides supporting documentation of activities in AllScripts and Weekend Log.
  • Attends scheduled supervision and obtains consultation when needed.
  • Performs all other duties as assigned.
