About The Position

Provides social services to medical patients and caregivers as they transition to the appropriate level of care. Completes psychosocial assessments for specific patient populations, including dialysis, mother-baby, and oncology patients, and collaborates with the interdisciplinary team on discharge planning for high-risk patients. Participates in interdisciplinary rounds and educates patients and caregivers, connecting them with appropriate community resources and agencies to help ensure safety upon discharge. The master's-prepared social worker functions within the guidelines of the National Association of Social Workers and is responsible for continued professional and personal growth.

Requirements

• Graduate from an accredited School of Social Work with a Master of Social Work (MSW) degree.
• Current New Jersey licensure as a Social Worker; an LSW is required.
• Excellent writing skills and effective communication skills.
• Ability to function well as part of a team.

Nice To Haves

• At least one year of post-master's social work experience in a healthcare setting is preferred.
• LCSW licensure may apply depending on years of social work experience and clinical courses.
