This role provides supportive services and counseling on healthcare and home care programs and services. As a member of the interdisciplinary team, the Social Worker assists with social, emotional, and economical concerns of patients/clients/residents and their families/caregivers to help them achieve or maintain an optimal level of functioning by coordinating and planning programs. The Social Worker provides crisis intervention, assists families in understanding medical situations, and coordinates healthcare programs among patients, families, and psychosocial/healthcare teams. They respond to suspected abuse, neglect, or violence, develop appropriate plans of care, and provide referrals and information about available programs and services. Professionalism is demonstrated through participation in care conferences, transitional rounding, and advocacy. The role requires critical thinking, problem-solving, decision-making, and strong written and verbal communication skills to build rapport with patients, families, and healthcare teams. Depending on the department, social services may be provided for donors and transplant recipients.
