Requirements

  • Masters degree in Social Work.
  • Texas licensed Master Social Worker.
  • Must have effective verbal/written communication skills and must have basic computer skills.
  • Must demonstrate knowledge and skill in the evaluation of patient needs

Nice To Haves

  • One year of social work experience preferred.

Responsibilities

  • Coordinates the discharge planning process, provides patient and family counseling, crisis intervention, financial needs assessment, information and referrals utilizing appropriate hospital department and/or community resources.
