About The Position

The social worker is responsible for assessing the patient’s psychosocial-spiritual needs, education and counseling. The social worker attends various medical staff or other departmental and committee meetings as required in representing patient interests and/or concerns. They administer patient assessment and write evaluation reports recommending treatment based on patient’s needs and function level. The social worker involves family in the treatment and discharge planning process if agreed upon by the patient, and records patient progress notes in their respective charts. They are active in the patient discharge planning process and assist in the patient’s discharge to home or other extended care facility placement. If a Master Social Worker, they may provide supervision to Bachelor Social Worker staff as needed. They collaborate with outside agencies to coordinate continued care, and select and schedule treatment activities consistent with the changing needs of the patient. The social worker coordinates with other team members in the implementation of treatment plan and communicates recommendations to other disciplines. They will keep abreast of new regulations, procedures, equipment and methods, and evaluate patient goal and objective accomplishment, presenting verbal and written criteria for termination of treatment. They must be knowledgeable of the Hospital’s computer system as it relates to the Social Services department and accessing patient related information. They must possess excellent interpersonal and communication skills with the ability to deal with confidential information tactfully and diplomatically. They will assist with the legal process as it relates to involuntary patients or guardianship. They must be able to use a personal computer, calculator, copy/fax machine, and other standard business office machines. They participate in Continuous Quality Improvement as required by the Medical Center and the job description, and understand and are accountable for the Medical Center's customer service program.

Requirements

  • MSW: Masters of Social Work
  • Limited License MSW employees must be actively working to obtain full licensure.
  • These employees must pass licensing exam and work 4,000 hours supervised by a LMSW.
  • Must be able to use a personal computer, calculator, copy/fax machine, and other standard business office machines.
  • Excellent interpersonal and communication skills with the ability to deal with confidential information tactfully and diplomatically.
  • Ability to handle multiple tasks, get along with others, work independently, regular and predictable attendance and ability to stay awake.
  • Overall dexterity is required including handling, reaching, grasping, fingering and feeling.
  • Overall vision and hearing is necessary with or without assisted device(s).
  • Is able to move freely about facility with or without an assisted device and must be able to perform the functions of the job as outlined in the job description.

Nice To Haves

  • Certification in Case Management preferred.
  • Recommended that the Case Manager receives their ACM certification after two years of experience.
