About The Position

Centerpoint Health—Paris, part of Lifepoint Health, is seeking a compassionate and dedicated Social Worker to join their team on a PRN (as needed) basis. This role is vital in making communities healthier by providing exceptional care. The Social Worker will be part of a dedicated team that shares a mission and culture of safe and compassionate care.

Requirements

• Current bachelor's in social work (required).
• Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire.
• Handle With Care within 30 days of hire.

Nice To Haves

• Masters degree preferred.

Responsibilities

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress.
• Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.
