About The Position
The Social Worker provides medical social services under the direction of a physician and Interdisciplinary Group. The role helps patients and their families understand significant social and emotional factors related to the patient’s health status and develop coping mechanisms. The Social Worker contributes to the patient’s Plan of Care and may function as Bereavement Coordinator, supervising bereavement services and establishing plans of care that address bereavement needs.
Requirements
- Master’s degree from a school of social work accredited by the Council of Social Work.
- At least 1 year of social work experience in a health care setting.
- Current driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
Nice To Haves
- Licensed Social Worker in the state of practice.
- At least 1 year of social work experience in a health care setting.
- Current CPR certification or ability to complete certification within 90 days of hire.
- Bereavement coordination experience.
- Experience establishing a Plan of Care for bereavement needs.
Responsibilities
- Help patients, significant others, physicians, and health care team staff understand personal, emotional, environmental, and social factors related to the patient’s health status, as needed.
- Contribute as a health care team member to the development of a comprehensive, integrated Plan of Care for patients.
- Instruct health care team members about community resources available to assist patients, as needed.
- When needed, function as Bereavement Coordinator and supervise the provision of bereavement services reflective of the patient’s family.
- Establish a Plan of Care that addresses bereavement needs and clearly delineates the services to be provided.
