About The Position

Sentara Health is seeking to hire a qualified individual to join our team as a Social Worker. The Social Worker BSW - Home Health performs a variety of casework and provides social work services to patients, families, and significant others. This role requires developing and monitoring multidisciplinary collaboration of services to patients where appropriate. The Social Worker will assist adult patients and their families with personal and environmental difficulties associated with their medical condition, up to and including at the time of terminal illnesses.

Requirements

Bachelor's Level Degree in Social Work (Required)
Valid Driver's License (Required)
BLS (Required within 90 days of hire)
1 year of social work experience (Required)

Previous Home Care/Hospice experience (Preferred)

Responsibilities
