About The Position

The Cancer Center Social Worker will provide all aspects of social services within the Cancer Center and CCRC outpatient service areas, as assigned by the CCRC leaders. Services will consist primarily of identifying and meeting the psycho/social, emotional, and/or financial needs of the patients; discharge planning and/or counseling; guidance and support either by direct service or through community resources.

Requirements

  • Bachelor’s degree required (from accredited school of social work preferred).
  • Current MN Social Work license (minimum licensure level-LSW).
  • Previous social work experience in a medical setting preferred.
  • Knowledge of public assistance programs, community resources, good interviewing techniques, and counseling skills.
  • Must demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his/her assigned unit.
  • Must demonstrate knowledge of the principles of growth and development over the life span.
  • Must be able to assess and interpret data about the patient’s status in order to identify each patient’s age specific needs and provide the care needed by the (neonate, pediatric, adolescent, adult, and/or geriatric) patient group.
  • Successful completion of the pre-employment health screening process required.
  • Must be able to attend the work site as scheduled.

Responsibilities

  • Conduct patient/resident assessments and develop individualized care and discharge plans in collaboration with patients, families, and care teams.
  • Provide short-term counseling, financial guidance, and referrals to community resources and support services as needed.
  • Deliver patient and family education tailored to individual learning needs, cultural preferences, and communication abilities.
  • Support staff development through training, continuing education, and participation in organizational programs and committees.
  • Assist with onboarding and mentoring of new employees and social work interns while maintaining compliance with professional and organizational ethics standards.
  • Perform other duties as assigned by leader.
