The Social Worker is responsible for participating in an interdisciplinary team approach to serve the needs of the Center population. This role provides therapeutic intervention to help residents cope with the social and psychological aspects of their illnesses, impairments, or disabilities, as well as their feelings about institutionalization and separation from family, community, and loss. The Social Worker will maintain written documentation in the resident medical record, perform initial psychosocial needs assessments, and develop the social service component of the resident's plan of care. They will also participate in interdisciplinary care planning conferences, ensure resident and family input into the care planning process, and act as a liaison between residents, families, outside agencies, and the Center Administrator. Responsibilities include conducting family meetings, facilitating discharge planning, ensuring customer safety, coordinating admissions through referrals, developing relationships with referral sources, verifying insurance coverage, and conducting facility tours.
