About The Position

Under the direction of the Social Work Supervisor, and collaborating closely with clinical case managers and care teams, the Regal North Social Worker is responsible for providing social work services to all members within the Regal North region (San Fernando and Simi Valley campuses), as needed. This individual will provide individualized assessment and case management to Regal North members who are at high risk for hospitalization and those with complex social and health-management needs. The Regal North Social Worker will engage and support members both in-person and telephonically, and develop care plans in collaboration with outpatient care management teams. The Regal North Social Worker will provide knowledge, expertise, and connection to resources that are specific to the geographic region. The Population Health department employs a portfolio of programs to promote access to high-quality healthcare and social services with the goal of improving health outcomes and reducing preventable healthcare costs. Using a patient-centered approach, the Regal North Social Worker will collaborate closely with the other disciplines and programs to effectively manage members’ medical, psychosocial and environmental needs.

Requirements

  • Licensed LCSW or MSW
  • High level knowledge of Case Management processes and
  • Knowledge Medical, HMO, Medicare and Cal MediConnect covered services

Responsibilities

  • Work with healthcare providers, case management, administrators, and other individuals involved in member care to promote appropriate social services and medical care coordination.
  • Provide psychosocial assessment of Regal North members through home-, community- and facility-based visits, as appropriate.
  • Consult upon requests and/or accept social work referrals from the regional medical directors, case management, pharmacy team or PCP offices, regarding social service resources or information.
  • Provide community resources related to food, housing/homeless prevention, limited financial resources, fall risk preventions, transportation, and utilities assistance.
  • Provide other resources, when appropriate, related to: family/social support; parenting/educational skills, domestic violence, elder care, caregiver resources, and respite.
  • Assist members and/or their families in introducing and/or transitioning to palliative or hospice care, when appropriate.
  • Assist members with advanced care planning, including providing education and discussion of end of life care options.
  • Collaborate with regional teams, medical management, case management, and other professionals to identify and mitigate social drivers of poor health outcomes and preventable acute care utilization.
  • Participate in the implementation of psychosocial and health management interventions for members, as appropriate and as outlined by department protocols.
  • Attend and contribute to Interdisciplinary Care Team meetings regarding care to/for specific member, including interpreting psychosocial implications of illness, as needed.
  • Assist with referrals to behavioral health services, including: individual therapy, family therapy, group therapy, anger management, behavior management, suicide prevention, and alcohol/substance abuse.
  • Function as liaison between administration, patients, physicians, and other healthcare providers, when appropriate, while interacting professionally and keeping parties informed.
  • Identify and respond to psychosocial, cultural, ethnic, and religious/spiritual needs of members and their families, ensuring to treat them with respect and dignity.
  • Protect privacy for both members and employees; ensuring all personal health information is kept confidential and in compliance with HIPAA regulations.
  • Appropriately document all pertinent information in the electronic medical documentation system.
  • Maintain accurate data tracking for program and departmental evaluation purposes.
  • Attend and participate in department meetings.
  • Pursue additional education (in-services, seminars, self-study, and formal education programs) to advance skills and knowledge.
  • Assist in educational endeavors of department.
  • Participate in orientation and/or instruction/training of new personnel.
  • Perform other duties as assigned.
