About The Position

CBI Home Health is looking to hire a Social Worker in Hawkesbury region. As a Social Worker in our Integrated Care Program, you’ll support smooth transitions home by addressing psychosocial needs, connecting patients to community resources, and collaborating with an interdisciplinary team to improve outcomes and reduce readmissions.

Requirements

• Bachelor of Social Work or a Master of Social Work
• Registered member in good standing with the Ontario College of Social Workers and Social Service Workers
• Passionate, empathetic, use good judgement and apply critical thinking when making client decisions
• Excellent communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
• Bilingual language skills in English and French are a MUST have

Nice To Haves

• 1 to 2 years of community experience is an asset
