About The Position

As a key provider of psychosocial care, you will conduct thoughtful, comprehensive assessments; partner with the medical team to create and implement person-centered plans; and connect patients and families with community supports that reduce stress and promote recovery. You will deliver services consistent with organizational policies and applicable regulations while helping to shape positive patient experiences and outcomes.

Requirements

  • Master’s Degree in Social Work
  • Completion of an approved internship program (completion of JHACH intern program or a comparable external clinical internship)
  • Strong listening skills and excellent verbal and written communication abilities
  • Proficiency documenting in electronic medical records and attention to accurate recordkeeping

Responsibilities

  • Perform comprehensive psychosocial assessments to identify and address patients’ and families’ social, emotional, and practical needs that affect treatment and recovery
  • Conduct suicide risk assessments and recommend appropriate safety measures to the interdisciplinary team
  • Coordinate referrals and collaborate with multidisciplinary colleagues to link patients and families to community and hospital resources that support continuity of care
  • Provide targeted therapeutic interventions, including crisis intervention, grief and supportive counseling, strength-based problem solving, psychoeducation, and strategies to enhance healthy coping
  • Work closely with medical staff to develop patient-centered care plans and identify psychosocial barriers that may delay or prevent discharge
  • Maintain timely, accurate electronic documentation and data collection in the medical record
  • Communicate consistently with team members about patient needs and interventions; actively participate in team meetings, huddles, and rounds
