Requirements
- Graduate of a master’s program in social work accredited by the Council on Social Work Education.
- Current licensure in the state of Michigan. - Cannot accept Limited License MSW
- At least one (1) year of Home Health experience required.
- A genuine passion for making a difference in people’s lives.
Responsibilities
- Assess the psychosocial needs of patients and their environments, sharing findings with the care team.
- Conduct social evaluations and create intervention plans based on findings.
- Maintain detailed clinical records for all referred patients.
- Prepare comprehensive clinical and progress notes.
- Provide patients and families with resources and referrals to address practical and environmental needs.
- Deliver care with passion and compassion in every interaction.
