About The Position

As a valued member of the interdisciplinary care team, the Social Worker is responsible for assessing, evaluating, and addressing the psychosocial needs of pediatric patients and their families. Using professional judgment, clinical expertise, and a collaborative approach, the Social Worker develops and implements care plans that promote patient safety, well-being, and positive outcomes. The Social Worker champions Patient- and Family-Centered Care principles by advocating for patients, providing emotional support, and connecting families with appropriate community and healthcare resources. Responsibilities include delivering direct social work services, coordinating care, educating patients, families, and healthcare team members, and facilitating access to support services. This role requires proactive assessment and anticipatory planning to identify and address potential patient and family needs before challenges arise. The Social Worker works closely with physicians, nurses, and other healthcare professionals to ensure resources are utilized effectively and efficiently while supporting quality care delivery. The Social Worker is expected to maintain professional competencies, participate in ongoing education and development activities, and engage with community organizations and professional associations to enhance patient care and support community health initiatives.

Requirements

• Bachelor's degree or higher in Social Work, or bachelor's degree in related field.
• Experience with individuals who have special health care needs.
• 2 years or more of clinical experience as a Social Worker.
• Licensed Social Worker (LSW) Current and valid license to practice in the State of MN required.
• Critical thinking and organizational skills.
• Ability to work collaboratively with multidisciplinary team.
• Excellent oral and written communication.

Nice To Haves

• Master's degree preferred.
• Epic experience is a plus.
