Requirements

Meets all state required regulations to practice in the dialysis setting
Master's degree in Social Work (MSW) required with a specialization in clinical practice
Demonstrated knowledge of government and private insurance programs
Basic computer skills in MS Word, Excel, PowerPoint and Outlook
Functional proficiency with DaVita specific applications within 60 days

Responsibilities

Support and advocate for patients
Help patients understand their rights and responsibilities
Guide patients in managing the physical, mental, emotional and financial demands of End Stage Renal Disease
Build meaningful and long-term relationships with patients and their families in an intimate outpatient setting
